(1) Participation in SDS is voluntary. The participant must:

1. (a) express an interest to participate in SDS and choose services through available service models. A participant may self-direct some or all of their services;
2. (b) participate, along with their chosen planning team, to the fullest extent possible in person-centered planning and development of the ISP as defined in 115 CMR 6.20 through 6.25;
3. (c) implement the person-centered plan and the ISP, including the hiring or purchasing of necessary services, supports or goods;
4. (d) utilize other available resources, services, supports, and goods, including natural supports, generic services, as those defined in 115 CMR 2.01: Definitions, and those available through MassHealth or other state agencies, in order to meet the participant's assessed needs; and
5. (e) work collaboratively with the Department and the FMS entity.

(2) Responsibility of the Department.

1. (a) The service coordinator shall, at the time of the planning meeting and as a part of the ongoing planning process:

   1. 1. facilitate and assist in the preparation of a person-centered plan, ISP, and individual budget for each participant and, if applicable, the selection of a financial management service and independent facilitator;
   2. 2. collect and disclose information to participants about providers and vendors who provide goods and services to participants; and
   3. 3. provide information to the participant regarding the SDS and the requirements for participation set forth at 115 CMR 14.05(1).
