The worker shall establish the identity of the applicant. Proof of identity shall be by:
(A) a birth certificate;

(M) in the event that none of the above are available at the time of application, identity may be temporarily verified by a signed and dated third-party sworn statement from an authorized person from one of the following types of organizations:
steps to obtain permanent verification of identity (see 106 CMR 703.810(A) through (M)) within the application time frames. Unreasonable failure to do so shall result in the denial or termination of assistance; no additional periods of eligibility may be established until such permanent identity verification is provided.
