(A) Grounds for denial of accreditation include, but are not limited to, the following:

1. (1) Failure to meet the requirements for becoming accredited by the Department, in accordance with 105 CMR 170.946;
2. (2) Failure to submit an application and fee in accordance with 105 CMR 170.946(C);
3. (3) Failure to satisfy the Department as to any of the grounds for determining the responsibility and suitability of the applicant under 105 CMR 170.948; or
4. (4) Fraud, deceit or knowing submission of inaccurate or incomplete data to the Department, either orally or in writing.
