(A) Grounds for license revocation, suspension, or refusal to renew a license include, but are not limited to, the following:

1. (1) Violation of any applicable requirement prescribed under M.G.L. c. 111C;
2. (2) Violation of a correction order;
3. (3) Engaging in, or aiding, abetting, causing, or permitting any act prohibited under M.G.L.
4. c. 111C;
5. (4) Failure to allow the Department to inspect the service or parts thereof;
6. (5) Use of EMS personnel who are not certified or not qualified to carry out the required level of patient care;
7. (6) Failure to comply with a plan of correction;
8. (7) Operation or maintenance of an EMS vehicle, EMS equipment or service in a manner that endangers the public health or safety;
9. (8) Failure to meet the licensure requirements of 105 CMR 170.000;
10. (9) Conviction of the licensee or a person with a significant financial or management interest in the service of Medicare or Medicaid fraud or other criminal offense related to the operation of the service;
11. (10) Conviction of the licensee or a person with a significant financial or management interest in the service of a criminal offense such as drug-related offenses, rape, assault, or other violent crime against a person, which indicates that continued operation of the service may endanger the public health or safety;
12. (11) Failure to comply with 105 CMR 170.303 regarding M.G.L. c. 94C registration;
13. (12) Failure to submit an acceptable plan of correction as required under 105 CMR 170.710;
14. (13) Failure to pay a deficiency assessment levied in accordance with 105 CMR 170.730;
15. (14) Failure to provide certification of compliance with all laws relating to taxes, or to provide a certificate of good standing from the Commissioner of Revenue, in accordance with M.G.L. c. 62C, § 49A;
16. (15) Any attempt to impede the work of a duly authorized representative of the Department or the lawful enforcement of any provisions of M.G.L. c. 111C or 105 CMR 170.000; or
17. (16) Any attempt to obtain or maintain a license or certificate of inspection by fraud, misrepresentation or the submission of false information.
