1. (A) The Department shall issue licensed to qualified applicants. Said licensed, unless previously suspended or revoked, shall expire on the first day of October of each even numbered year following the date of issuance of the original license or shall expire not more than two years from date of issue as determined by the department. On or before the first day of August each even-numbered year or at least 60 days prior to the expiration date, the Department shall mail an application for renewal of license to every person to whom a license has been issued or renewed during the current licensure period. License renewal applications shall be filed with the Department, together with a non-refundable renewal

fee payable to the Commonwealth of Massachusetts, on or before the 15 day of September each even-numbered year or 30 days prior to the expiration date of the license.

1. (B) Applicants for license renewal shall submit evidence of all continuing professional education required to maintain the desired level of licensure as specified in 105 CMR 168.015.
2. (C) Failure to comply with these requirements shall constitute grounds for non-renewal or expiration of a license. If such action is taken, the Department shall provide the applicant with written notice of the Department’s decision stating the reasons of the denial.
