The temporary nursing service agency shall maintain a record for each of the medical personnel it assigns to health care facilities. Such record shall include:
- (A) evidence of current registrations, professional licensure or certification as applicable;
- (B) evidence that the individual has completed pre-employment physical and testing for communicable disease as applicable;
- (C) evidence of a background or reference check;
- (D) copies of reports required under 105 CMR 157.220(B)(6)(b) and 157.600; and
- (E) copies of annual performance reviews required under 105 CMR 157.500.
