Each person who owns or possesses and administratively controls a laser facility, unless specifically exempted in 105 CMR 121.014 shall:

1. (A) Apply for registration of such facility with the Radiation Control Program prior to the operation of a laser facility. Application for registration shall be completed on forms furnished by the Radiation Control Program and shall contain all the information required by the form and accompanying instructions.
2. (B) Designate an individual to serve as the Laser Safety Officer.
