1. A. Name and addresses of the applicant and applicant's parents or guardian.
2. B. Name and address of recommending supervisor of the project.
3. C. Personal financial statement of the applicant, signed by the applicant and parents or guardian, on forms provided by the commission.
4. D. A budget reflecting estimated expenses, income and repayment of the loan for the term of the loan.
5. E. Signed statements requested of the recommending supervisor and parents or guardian as required in §1503.
6. F. The name of the bank and the account number to which loan funds shall be deposited and authorized signatures for the withdrawal of funds.
