1. A. Admission Data

   1. 1. The provider shall maintain accurate records on all new admissions, to include the following data fields:

      1. a. demographics of youth admitted, aggregated by:

         1. i. race;
         2. ii. ethnicity;
         3. iii. gender;
         4. iv. date of birth;
         5. v. parish of residence; and
         6. vi. geographical zone determined by provider to include zip code, local law enforcement zones);

      2. b. legal status of youth, aggregated by:

         1. i. custody status of the youth; and

         2. ii. adjudication status;

            1. (a) pre-adjudicated-status/delinquent; and
            2. (b) post-adjudicated-status/delinquent;

      3. c. offenses of youth admitted, aggregated by:

         1. i. specific charge(s);
         2. ii. intake date; and
         3. iii. release data;

2. B. Operational Data

   1. 1. The provider shall maintain accurate records of operational events that include the following data fields:

      1. a. youth released, aggregated by:

         1. i. race;
         2. ii. ethnicity;
         3. iii. gender; and
         4. iv. custody status;
      2. b. average daily population of youth in the facility; and
      3. c. average length of stay of youth in the facility.

3. C. Detention Screening Data

   1. 1. The provider shall maintain accurate records on all risk assessment instrument (RAI) on new admissions conducted, to include the following data fields:

      1. a. demographics of youth screened, aggregated by:

         1. i. race;
         2. ii. ethnicity;
         3. iii. gender;
         4. iv. date of birth;
         5. v. parish of residence; and
         6. vi. geographical zone determined by provider to include zip code, local law enforcement zones;

      2. b. offense of youth screened:

         1. i. specific charge(s); and
         2. ii. release date;

      3. c. screen data:

         1. i. date completed;
         2. ii. overrides usage; and
         3. iii. screening outcomes: release/alternative to detention/secure detention;

      4. d. outcome data:

         1. i. successful/unsuccessful; and
         2. ii. recidivism/failure to appear (FTA).
