1. A. Applications for a license from an employee of a certified firm shall be on forms provided by the State Fire Marshal and accompanied by the required fee as specified in these rules.
2. B. Applications for employee licenses shall be accompanied by a written statement from the employer certifying the applicant's competency to certify, hydrostatically test, inspect, maintain or service those systems and/or equipment for which the applicant desires to become licensed.
3. C. Applications for employee licenses will not be accepted unless accompanied by documentation showing that the applicant has met all competency requirements as determined by the Life Safety and Property Protection Advisory Board.
