1. A. Requirements for reclassifying a qualifying vehicle as a utility terrain vehicle:

   1. 1. a completed vehicle application form (DPSMV 1799), including section D on the reverse side of the form;
   2. 2. original title;

   3. 3. a statement from the purchaser indicating the following:

      1. a. that the utility terrain vehicle contains all equipment required for use as indicated above and has a minimum wheelbase of 61 inches but not exceed 110 inches;
      2. b. that the owner is aware that the vehicle is only permitted to be used by a licensed driver at least 21 years of age;
      3. c. that the utility terrain vehicle may only be used upon a parish road that has been designated by a parish, or a municipal street that has been designated by a municipality, for use by a utility terrain vehicle or cross any divided highway, highway, roadway, or street with a posted speed limit in excess of 35 miles per hour at an intersection;
   4. 4. applicable fees (title and handling).
