1. A. The manufacturer, or his designated representative in Louisiana, shall maintain in a file unique to each probationary driver legible copies of the following documents:

   1. 1. order of the sentencing court setting forth the terms and conditions of probation;
   2. 2. certificate of installation and acknowledgment by probationary driver;
   3. 3. ignition interlock monitoring reports;
   4. 4. if the probationary driver is utilizing a third-party owned vehicle, written permission from the owner of said vehicle to install the approved ignition interlock device;
   5. 5. any physical evidence of tampering or attempted tampering with the ignition interlock device discovered by the manufacturer or his designated representative.
