1. A. The laboratory shall have a procedural manual which will include detailed descriptions of procedures for testing. In addition, laboratory policies shall exist which govern sample receiving, chain of custody, analysis, quality control and quality assurance, choice of reagents, review of data, and reporting. The procedural manual shall include the following:

   1. 1. instructions for preparation of reagents;
   2. 2. details of the analytical procedure;
   3. 3. instructions for preparation of controls and calibrators;
   4. 4. references to include theory and principle of the method.
