1. A. Disclosure forms filed pursuant to R.S. 27:12(B) shall:

   1. 1. be in writing and on a form provided by the board or a form which is substantially similar to the form provided by the board;
   2. 2. filed before confirmation of the Gaming Control Board member and annually thereafter no later than January 31 of each calendar year;

   3. 3. contain:

      1. a. the name and address of the member;
      2. b. the position held by the member;
      3. c. all assets and liabilities, property and business interests, and sources of income of the member, the spouse of the member and the minor children of the member; and
      4. d. a sworn affidavit as to its accuracy.
2. B. The executive secretary shall maintain these forms suitably indexed.
