Disclosure Forms Filed Pursuant to R.S. 39:1233.1

LR 30:2670 (December 2004)Promulgated in accordance with R.S. 42:1134(A).State of Louisiana

A. Disclosure forms filed pursuant to R.S. 39:1233.1 shall:
1. 1. be in writing and on the form in §1910;
2. 2. contain:
  1. a. the name and address of the public servant;
  2. b. the public position held by the public servant;
  3. c. the name and address of the bank;
  4. d. the position held with the bank by the public servant and whether that position is compensated or noncompensated; and
  5. e. a description of the transaction from which the public servant recused himself from participating; and
3. 3. be signed by the public servant.
B. The executive secretary shall maintain these forms suitably indexed.
