A. [Formerly paragraph 28:020-1] The following fees shall accompany each application for initial registration.
1. 1. Make check or money orders payable to the Department of Health and Hospitals.
Registrant Fee
Owner of Facility $1,000
Manager of Facility $ 200
Operator $ 100
Training Facility or Person $3,000
