1. A. Team Minimum Requirements. The implant team shall be composed of the following members at a minimum:

   1. 1. physician/otologist;
   2. 2. audiologist;
   3. 3. speech/language pathologist;
   4. 4. psychiatrist; and
   5. 5. educator of the deaf with experience in oral/auditory instruction.
