1. A. Documentation

   1. 1. Documentation for EPSDT PCS provided shall include at a minimum, the following:

      1. a. documentation of approval of services by BHSF or its designee;
      2. b. daily notes by PCS provider denoting date of service and services provided (checklist is adequate);
      3. c. total number of hours worked;
      4. d. time period worked;
      5. e. health condition of beneficiary;
      6. f. service provision difficulties;
      7. g. justification for not providing scheduled services; and
      8. h. any other pertinent information.

   2. 2. There shall be a clear audit trail between:

      1. a. the prescribing practitioner;
      2. b. the personal care services provider agency;
      3. c. the person providing the personal care services to the beneficiary; and
      4. d. the services provided and reimbursed by Medicaid.

2. B. Agencies providing EPSDT PCS shall conform to all applicable Medicaid regulations as well as all applicable laws and regulations by federal, state, and local governmental entities including, but not limited to:

   1. 1. wages;
   2. 2. working conditions;
   3. 3. benefits;
   4. 4. Social Security deductions;
   5. 5. OSHA requirements;
   6. 6. liability insurance;
   7. 7. Workman’s Compensation; and
   8. 8. occupational licenses; etc.
