   8. 8. physical therapy and rehab evaluation—$75;
   9. 9. physical therapy, one modality—$37;
   10. 10. physical therapy, two or more modalities—$56;
   11. 11. physical therapy, one or more procedures, and/or modalities, 15 minutes—$18.50;
   12. 12. physical therapy with procedures, 30 minutes—$37;
   13. 13. physical therapy with procedures, 75 minutes—$92.50;
   14. 14. procedures and modalities, 60 minutes—$74;
   15. 15. speech and hearing therapy, 15 minutes—$14;
   16. 16. speech and hearing therapy, 30 minutes—$28;
   17. 17. speech and hearing therapy, 45 minutes—$42;
