1. A. The Medicaid Program shall provide coverage for the administration of childhood and adolescent vaccines recommended by the Advisory Committee on Immunization Practices and available through the Louisiana Immunization Program/Vaccines for Children Program.

2. B. To qualify for Medicaid reimbursement for vaccine administration, a provider must be:

   1. 1. a licensed health care provider who has the authority under Louisiana state law to administer childhood and adolescent vaccines;
   2. 2. an enrolled Medicaid provider; and
   3. 3. an enrolled Louisiana Immunization Program/Vaccines for Children Program provider.
