1. A. The licensee and the casino operator shall maintain a division-approved surveillance log. The log shall be maintained by surveillance room personnel in the surveillance room. The division shall have access to the log at all times. A log entry shall be made in the surveillance log of each surveillance activity. Each log entry shall include the following:

   1. 1. the identity of any person entering and exiting the surveillance room;
   2. 2. a summary, including date, time and duration, of each surveillance activity;
   3. 3. a record of any equipment or camera malfunctions;
   4. 4. a description of any unusual events occurring; and
   5. 5. any additional information as required by the division.
2. B. The surveillance logs required by this Section shall be retained and stored by month and year.
