1. A. An application for a gaming employee permit shall be made on forms prescribed by the board or division and shall contain all information requested by the board or division. All applications are to contain a properly notarized oath wherein the applicant states that:

   1. 1. the information contained therein is true and correct;
   2. 2. the applicant has read the Act and these rules, and any other informational materials supplied by the division; and
   3. 3. the applicant agrees to comply with these rules and the Act.
2. B. An applicant for a gaming employee permit shall submit to fingerprinting at the direction of the division and supply a color passport size photograph. The photograph must be satisfactory to the division and must have been taken not earlier than three months before the date of filing the application.
3. C. The applicant shall also provide any other information requested by the division.
4. D. An applicant for a gaming employee permit shall pay the application fee established by the Act prior to the issuance of the permit.
5. E. Failure to comply with the provisions of this Section may constitute grounds for delaying consideration of the application or for denial of the application.
