1. A. The charitable organization and/or the private casino contractor conducting a casino night must prepare and maintain a register of workers including the following information:

   1. 1. name;
   2. 2. current residential address;
   3. 3. date of birth;
   4. 4. job description; and
   5. 5. name of organization or private casino contractor.
