1. A. The subgrant proposal shall be prepared in the format prescribed by the Department of Labor, and shall include (but not be limited to) the following:

   1. 1. identification of the eligible entity, to include the corporate name, street address and contact person;
   2. 2. a complete budget, including a budget summary, spending plan and staffing plan; and
   3. 3. a complete description of the programmatic activities to be funded which must provide programs in accordance with the CSBG Act.
2. B. The forms for submission of the subgrant proposal will be provided by the Department of Labor, CSBG Unit.
