1. A. The forms previously promulgated in Appendices B and C shall continue in use until May 30, 2020.
2. B. The amendments to Appendices B and C shall be used for all new and renewal policies issued or renewed on or after May 30, 2020.
3. C. The form previously promulgated in Appendix A remains unchanged and shall continue in full force and effect until amended, modified, altered or rescinded by the commissioner of insurance.
