1. A. Any eligible active or retired member of the Louisiana National Guard who submits an enrollment application to participate in OGB sponsored life, health, or other programs shall provide, contemporaneous with the enrollment application, written certification as follows:

   1. 1. from the member's employer, that other coverage is not available through the member's employment; and
   2. 2. from the appropriate federal administrative agency that the member is not eligible for Medicare;
   3. 3. OGB may require additional written certification at such times as it deems necessary to verify eligibility for participation in its sponsored programs.
