1. A. A certificate of completion of the six hour general training course shall be issued by the detention facility to those employees who successfully complete the course within the specified time lines. The certificate shall include at least the following information:

   1. 1. name of the unlicensed employee;
   2. 2. date of completion of the training and renewal date;
   3. 3. name of the juvenile detention center;
   4. 4. number of course hours;
   5. 5. signature of the instructor and the director of the detention center.
