LAC 19:VII.5105 – Application Process

A. The application shall include but not be limited to:
1. 1. a copy of the Phase 1 proposal to the federal program;
2. 2. a statement that the applicant has submitted the proposal to the federal program and that they are submitting a notification of intent to file with the state program;
3. 3. a use of funds schedule for the requested state grant;
4. 4. a copy of the grant/contract award from the federal program.
