1. A. LED will provide a standard application form which applicants will be required to use to apply for assistance under this program. The application form will contain, but not be limited to, detailed descriptions of the following:

   1. 1. justification of need;
   2. 2. the training provider's overall training plan, including a summary of the types and amounts of training to be provided and a description of how the training provider determined its need for training;
   3. 3. a preliminary budget, including but not limited to, proposed trainer salaries; and
   4. 4. any additional information LED may require.
