1. A. The department shall serve notice of intent to deny certification or renewal of certification, or to revoke an existing certification.
2. B. Such notice shall be a written determination by the department, including a brief statement of the reasons alleged for denial.
3. C. Such notice shall be served by certified mail or by mail service requiring a return receipt.

4. D. A denied applicant may request a hearing within 30 days of receiving notice, by filing a written request for hearing with the department.

   1. 1. Such hearing shall be conducted pursuant to the provisions of the Administrative Procedure Act, R.S. 49:950 et seq.
