1. A. An applicant for certification shall submit the following documents to the department:

   1. 1. complete, notarized certification application;

   2. 2. organizational management;

      1. a. certificate of incorporation;
      2. b. either 501(c)(3) tax exempt letter or CDFI fund certification letter;
      3. c. by-laws; and
      4. d. program of work;

   3. 3. board information;

      1. a. names and addresses of the partners, officers, directors or trustees and those principal owners or members;
      2. b. minutes of the four most recent board of directors meetings;

   4. 4. financial information;

      1. a. a copy of the most recent financial audit; and
      2. b. a copy of the fiscal budget, including year-to date expenditures;
   5. 5. business plan;
   6. 6. an application fee of $300;
   7. 7. any additional information requested by the department.
