LAC 10:XIX.121 – Preparation of Form UCC-3 Filing

A. Any Form UCC-3 filing changing the initial financing statement must:
1. 1. give the name and address (as applicable), of each debtor as it appears on the initial financing statement or the most recent filing;
2. 2. give the name and mailing address (as applicable), of the secured party of record;
3. 3. give the initial UCC file number (entry number), the date of filing, and the parish in which the initial financing statement was filed; and
4. 4. indicate the type of action requested (Only one type of transaction may be requested on any Form UCC-3.).
