The institutional level of the system shall consist of at least the following information:

1. (1) Date of institutionalization;
2. (2) Type of incoming action;

3. (3) If defendant sentenced to death:

   1. (a) First scheduled date of execution;
   2. (b) Date defendant executed;
   3. (c) Date sentence commuted in lieu of execution;
   4. (d) Sentence to which sentence of death was commuted;
4. (4) Date defendant released from institution;
5. (5) Type of release from institution;

6. (6) If the offender is released on parole:

   1. (a) Offense for which convicted;
   2. (b) Maximum expiration date;
   3. (c) Minimum expiration date;
   4. (d) Was the parole supervision fee imposed;
   5. (e) What was the amount actually imposed for the parole supervision fee;
   6. (f) What amount of the parole supervision fee was actually collected;
   7. (g) Was restitution ordered as part of conditions of the parole;
   8. (h) What amount of restitution was ordered;
   9. (i) What amount of restitution has been paid;
   10. (j) Was a victim impact statement presented to the parole board; did it favor the release of the offender;

8. (k) Did the prosecutor present a statement to the parole board; did it favor the release of the offender; and
9. (l) Did the victim or a representative of the victim appear before the parole board; did he favor the release of the offender; If the offender released on parole violates parole or is rearrested:
10. (a) What was the specific parole violation;
11. (b) Was the offender arrested for the violation;
12. (c) What was the outcome of the parole violation hearing;
13. (d) Was the offender reinstitutionalized;
14. (e) If arrested for a new criminal offense, list the KRS number, name, and level of the offense;
15. (f) Was the offender subsequently convicted thereof;
16. (g) Was the offender reinstitutionalized for the offense; and
17. (h) Was the offender placed on probation for the offense.
