The court disposition level of the system shall consist of at least the following information as relates to the offenses charged and the offenses disposed of:

1. (1) The court which disposed of the offense;
2. (2) The name of the presiding judge;
3. (3) Date upon which the trial began;
4. (4) The final pleading, as to each charged offense and the level thereof;

5. (5) Listing of charged offenses disposed of by the court together with:

   1. (a) Date of disposition;
   2. (b) Listing of each offense and the KRS number, name and level;
   3. (c) Whether the offense was dismissed in exchange for a guilty plea on another offense; and
   4. (d) Number of offenses disposed of by the court;
6. (6) Listing of offenses for which the defendant was convicted by KRS number, name and level;
7. (7) Number of convicted offenses; and
8. (8) Date of conviction.
