The court disposition level of the system shall consist of at least the following information as relates to counsel:

1. (1) Did the defendant have counsel;
2. (2) Did the defendant retain private counsel;
3. (3) Was counsel provided by the public advocate;

4. (4) If counsel was provided by the public advocate:

   1. (a) Was reimbursement sought;
   2. (b) What was the amount of reimbursement sought;
   3. (c) What was the amount of reimbursement paid; and
   4. (d) What amount was recovered from bail money;
5. (5) Did the defendant have more than one (1) counsel; if so, the number of counsel.
