Application: contents.

All applications for an interstate motor fuel user license shall be on forms furnished by the director and shall contain information as follows:

(a) The name and address of the applicant.
(b) The principal office or place of business of the applicant.
(c) If a corporation, the names and addresses of the principal officers.
(d) The financial condition of the applicant.
(e) A description of applicant's operations.
(f) Such other information as the director shall require.
