Any student may elect coverage under the student health insurance program for any of the following sets of people, to the extent that the coverage is offered by the state board:

1. (a) The student;
2. (b) the student and the student's spouse;
3. (c) the student and any dependents; or
4. (d) the student, the student's spouse, and any dependents.
