To be acceptable for filing, an application for temporary permit to appropriate water for beneficial use shall meet the following requirements: (a) Be made on the form prescribed by the chief engineer;

1. (b) be signed by the applicant or an authorized representative of the applicant;
2. (c) be accompanied by the statutory application fee;
3. (d) contain all the information requested for the proposed use as set forth in the prescribed application form; and
4. (e) include any other information requested by the chief engineer that is necessary to understand the application.
