The collective bargaining unit shall file its petition in quadruplicate (4) in form as prescribed by the commissioner. Said petition shall contain:
- (a) Evidence of all-union agreement authorization by at least thirty percent (30%) of the employees in the appropriate collective bargaining unit.
- (b) An alphabetical listing of the names and addresses of the employees in the appropriate collective bargaining unit.
- (c) Three (3) copies of the current or most recently expired contract between the employer and the collective bargaining unit.
