1. (a) A docket of disciplinary cases shall be maintained, showing the following:

   1. (1) The case number;
   2. (2) the inmate's name;
   3. (3) the inmate's number;
   4. (4) the cell house;
   5. (5) the offense and its classification; and

   6. (6) the name and title of the reporting officer.

      Space shall be left on each line on the docket to enter the plea of the inmate, the findings of the hearing officer, and the sentence imposed.

2. (b) A copy of this docket shall be maintained in the facility.

This amendment shall be effective on and after February 15, 2002.
