Each order assessing a civil penalty shall include the following: (a) A general statement citing the section of the act authorizing the assessment of a civil penalty;

1. (b) a specific reference to each provision of the act or implementing regulation that the respondent is alleged to have violated;
2. (c) a concise statement of the factual basis for each alleged violation;
3. (d) the amount of the civil penalty; and
4. (e) notice of the respondent's right to request a hearing.
