1. (a) Non-commercial, non-emergency medical transportation providers shall be paid 22 cents per mile.

2. (b) Each commercial, non-emergency medical transportation provider shall be reimbursed at one of the following rates:

   1. (1) For level one general transportation, $10.00 for each one-way trip to a medicaid-covered service for a medicaid beneficiary, plus $1.00 per mile after 10 miles; or
   2. (2) for level two transportation for a non-ambulatory medicaid beneficiary, transportation of medical equipment with a medicaid beneficiary, or transportation of a medicaid beneficiary following a treatment that will result in a disabling physical condition, $20.00 for each one-way trip to a medicaid-covered service for a medicaid beneficiary plus $1.00 per mile after 10 miles.
3. (c) Reimbursement for necessary meals and lodging may be allowed for Kan Be Healthy participants and one attendant, subject to prior authorization.
4. (d) This regulation shall be effective on and after July 1, 2003.
