The term medical information as used in this regulation shall be considered to include but not limited to the following:

1. (1) Discharge summaries.
2. (2) Laboratory reports.
3. (3) X-ray reports.
4. (4) Diagnostic reports.
5. (5) Physical, psychiatric or psychological reports or general medical reports.
6. (6) Social histories.
