1. (a) An assessment upon admission for each resident shall be completed and shall include, but not be limited to a statement of presenting problems.

2. (b) Prior to the development of the mental health plan of care, a comprehensive assessment shall be completed within 14 days after admission for each resident and shall include but not be limited to:

   1. (1) The minimum data set plus prepared by a registered nurse;

   2. (2) psychosocial assessment prepared by a mental health professional to determine strengths and weaknesses in:

      1. (A) Living arrangements;
      2. (B) financial resources;
      3. (C) vocational and educational skills;
      4. (D) leisure pursuits;
      5. (E) social support structures; and
      6. (F) previous compliance with treatment programs; and

   3. (3) a social history which includes:

      1. (A) Family background;
      2. (B) educational history, and
      3. (C) employment history.
