In addition to the reporting requirements for risk management required in K.A.R. 28-34-142, each licensee shall ensure that the following incidents are reported to the department, on a form provided by the department:

1. (a) Each incident resulting in serious injury of a patient or a viable unborn child shall be reported to the department within 10 days after the incident.
2. (b) The death of a patient, other than the death of an unborn child, shall be reported to the department not later than the next department business day.
