1. (a) No earlier than 90 days before but no later than the renewal date, each licensee wishing to renew the license shall submit the following:

   1. (1) The nonrefundable license fee of $500; and
   2. (2) an application to renew the license on the form provided by the department.

2. (b) Each licensee shall submit a request for an amended license to the department within 30 days after either of the following:

   1. (1) A change of ownership by purchase or by lease; or
   2. (2) a change in the facility's name or address.
