Each person applying for registration as a home inspector shall provide the following to the board:

1. (a) The applicant's full legal name and date of birth;
2. (b) the applicant's residential, business, and mailing addresses;
3. (c) the applicant's residential and business telephone numbers;
4. (d) the applicant's electronic mail address;
5. (e) complete answers to all questions regarding conduct that might be grounds for denying an application for registration;

6. (f) (1) Identification of the school district or other entity that granted the applicant a high school diploma or its equivalent; or

   1. (2) documentation that the applicant was engaged in the practice of performing home inspections on July 1, 2009;

      (g)(1) Documentation that the applicant has completed an educational program that meets the requirements of K.A.R. 130-3-1; or

   2. (2) documentation that the applicant was engaged in the practice of performing home inspections for at least two years before July 1, 2009 and has completed at least 50 fee-paid home inspections;
7. (h) documentation from an insurer authorized to do business in Kansas stating that the applicant is insured by a policy of liability insurance in the amount required by K.S.A. 58-4509, and amendments thereto;

8. (i) documentation from an insurer or financial institution establishing that the applicant is financially responsible as required by K.S.A. 58-4509, and amendments thereto;

   (j)(1) Documentation that the applicant has successfully completed the examination required by the board for registration; or

   1. (2) documentation that the applicant was engaged in the practice of performing home inspections for at least two years before July 1, 2009 and has completed at least 50 fee-paid home inspections;
9. (k) the initial registration fee specified in K.A.R. 130-2-1; and
10. (l) a declaration signed by the applicant under penalty of perjury under Kansas law stating that all information submitted is true and correct.
