Each certificate of registration shall show on its face in clear and concise language the following information:

1. (a) The legal name of the AMC;
2. (b) the certificate of registration number;
3. (c) the date of issuance;
4. (d) the date of expiration; and
5. (e) the signature or facsimile signature of the chairperson of the board.
