1. (a) Each applicant for a certificate or license and each person whom the executive director determines to be subject to a background investigation pursuant to the act or these regulations shall complete any application or disclosure forms requested by the commission staff and shall submit to being fingerprinted.
2. (b) Each person that is subject to investigation shall have a duty to cooperate with the commission during any investigation and to provide the information that the commission requests.
