1. (a) Any sponsoring organization may submit an application to the board requesting approval as a single-program provider.

2. (b) Each sponsoring organization seeking single-program provider approval shall submit a complete application at least 30 days before the requested offering that provides the following:

   1. (1) Course educational objectives that are oriented towards the enhancement of EMS practice, values, skills, and knowledge;
   2. (2) name of each qualified instructor for the course; and
   3. (3) date, title, and location of the course.
